We know from community interactions that many participants don’t realise that they may be eligible for Specialised Disability Accommodation.

At Horizon SDA Care we’d like to change that.

Are You Eligible?

You are eligible for the NDIS and Specialised Disability Accommodation if you:

  • Have a permanent disability that significantly affects your ability to take part in everyday activities or a developmental delay. To meet the NDIS disability rules you need to have an impairment or condition that is likely to be permanent (lifelong) and that stops you from doing everyday things by yourself. This includes conditions which can vary in intensity, such as mental illness.
  • Be aged less than 65 when they first enter the NDIS;
  • Be an Australian citizen or hold a permanent visa or a Protected Special Category visa; and
  • Live in Australia where the NDIS is available.

Registration

After carefully considering all of your options, ensuring you are serious about taking the next step in securing long term, stable and permanent accommodation that meets your specific needs, and you wish to formally apply for an HPA Care Home, register your interest by completing our expression of interest form.

Your allocated Project Manager will confirm receipt of your expression of interest and begin the process of working with you and your support network to select the right location, floor plan, house mates & Technology package.

Meet With Us

Once you have met with your Property Manager / NDIS planner and determined your eligibility for SDA funding, & completed your expression of interest we will arrange for our Project Manager to meet with you to discuss your accommodation preferences and requirements.Â

Your NDIS planner will have already discussed factors relating to your accommodation funding which will meet one of the following SDA design categories:

  1. Improved Liveability
  2. Full Accessibility
  3. Robust
  4. High Physical Support

Your meeting

When you meet with us, we will help you work through your options and the key factors of living in a HPA Care Home including:

Location preferences: What area/s are you willing to live?  The more flexible your requirements the greater number of options available to you – our footprint extends across Australia. To find out if the SDA roll out is happening if your area click here.

The House: We will run through the floor plans with you and make adjustments as required.

House mates: Do you have funding limitations relating to your support?

  • You may already know someone with similar support requirements who would be interested in sharing accommodation with you.

Support Services: All our accommodation includes either a sleep over room or a separate adjoined dwelling for support workers and is aimed at increasing your independence.

FAQ's

Who will decide if I can access the NDIS?

The NDIA determines eligibility for the NDIS. The NDIA is the national agency responsible for running the NDIS. Most people who receive NSW funded disability supports will go through a simplified eligibility process, based on the supports they currently receive now.

You can call 1800 800 110 to make an Access Request or you can ask for a form. If you need help filling in the form or making the call, you can contact your Local Area Coordinator, Early Childhood Early Intervention partner or your contact your local NDIA office
